CE Partner Agency Resources

​River Valleys CoC has developed detailed policies and procedures, helpful resources, and training opportunities for agencies and programs that participate in Coordinated Entry (CE) in our region. CE policies are maintained by the Coordinated Entry Committee and are approved by the CoC membership. Procedures and forms may be changed as needed by CoC staff to maintain alignment with approved CE policies. 

Participating agencies are required to use approved forms for completing assessments and receiving referrals. The most current version of forms will be posted here. ​

Policies

Policies define and guide our CE system. They answer why our CE system exists and what it is meant to do. 
Three primary sets of policies guide our CE system. Funder requirements for individual programs also affect how CE works.  

River Valleys CoC CE Policy

The Coordinated Entry Policy of River Valleys CoC was developed and approved by CoC members to guide agencies and individuals in implementing CE as intended by the community. 

The policy describes: 
  • How referrals are prioritized
  • Roles for assessors, housing providers, navigators, and others 
  • Expectations for data management and reporting
  • Values and standards for CE activities
  • and more

Minnesota's HMIS Policies 

Many parts of the CE system are managed within HMIS (Homeless Management Information System). The policies of Minnesota's HMIS further define how to use the data system and how decisions about the data system will be made.

Procedures

Procedures detail "how" we will implement the CE system that was agreed in policy, in a way that works for people across the multiple counties and through many agencies that are involved in the homelessness response system.  
Procedures are defined by role or activity: housing problem solving & assessment, housing provision, navigation, and case conferencing.

Sign up for training

Coordinated Entry 101
Get to know the purposes and processes of Coordinated Entry. Great for new staff, refreshers, supervisors of staff that work with CE, and other community partners. Offered monthly on the first Wednesday at 9-10am.

​Assessor Training 
​Training is required for anyone who will be conducting Housing Problem Solving and CE assessments. Offered upon request Prerequisites: CE 101 and HMIS login required. ​

​Housing Provider Training 
Training is required for any housing program advocate who will be requesting referrals from the CE priority list and working with clients on housing referrals. Offered monthly. Prerequisites: CE 101 and HMIS login required. ​

Monthly CE Assessor & Housing Provider calls 
Third Wednesdays at 9-10am 
Targeted training topics with time for questions.

River Valleys CoC is also a Continuum of Care region recognized by the U.S. Department of Housing and Urban Development for the purposes of the HUD Continuum of Care funding program. The CoC's formal name with HUD is Rochester/Southeast Minnesota CoC (MN-502).
